The federal government funds 18 hospitals as Spinal Cord Injury Model Systems for the 2021–2026 cycle — the NIDILRR/ACL centers recognized for exceptional SCI care and the research that shapes how spinal cord injury is treated everywhere else. If you're choosing where to get care, Model System designation is one of the strongest quality signals there is.

What "Model System" means. The designation comes from the National Institute on Disability, Independent Living, and Rehabilitation Research (NIDILRR), part of the Administration for Community Living. Centers earn these five-year grants through a competitive, peer-reviewed process by meeting detailed federal criteria — they have to deliver comprehensive, coordinated SCI care and run an active SCI research program, conducting their own studies and contributing to the national SCI database (the NSCISC, running since 1973). Most are based at major academic medical centers, so the designation usually comes with a robust level of SCI research — from the laboratory bench to clinical trials — which can mean earlier access to emerging treatments. 18 centers are funded in the current 2021–2026 cycle (14 awarded in 2021, with 4 more added in 2022). That cycle runs through August 31, 2026, and the roster can change when the next cycle is announced, so confirm a center's current status before you rely on it. You don't have to be treated at one to get excellent care — but they are the recognized national leaders, including several of the most famous SCI hospitals in the country (Craig, Shepherd, TIRR, Kessler, Shirley Ryan AbilityLab).
